After finishing the FTDI side I went back to re-implement the USB COM port, and it's all working fine now! I've even tried both native Win32 API and CRT, and both are working correctly. So I've no idea what the problem was with the old code.
Opening the port through the Win32 API did start in 7-bit mode, so I had to change that. Opening in binary mode for the CRT seemed to do all that's needed.
I've got more work to do on the reading side before I attempt writing. I might be back at that point...
